Given the titles, the music is probably from at least two different nights. The Davis Quintet was booked for four nights at the Jazz Workshop, December 5-8.
This was Williams's last live gig with the Davis Quintet. He remained with Davis through the February 18 In a Silent Way session. Although he was originally slated to accompany the Quintet on a Japanese tour in January 1969, the tour was canceled due to trouble with Japanese immigration officials. Jack DeJohnette replaced Williams for good in late February 1969.

(I know my tracks don't match this listing, if someone has the time to figure out what the 6th track is, be my guest.)
